Make your first appointment an easy one!

Appointment Checklist

• All copays or procedure prepays are due in full at the time of service
  Complete all requests sent by our online portal
• Bring your Insurance card(s)
• Valid driver’s license
• Any previous medical records, especially test results and medication lists
• If your insurance requires, please obtain a referral prior to your visit
• Plan to arrive at least 20 minutes before your scheduled appointment time

Friendly Reminders

Fill out your patient paperwork early!

We strongly recommend that patient paperwork is completed electronically prior to arrival. A link to complete your forms will be sent via email 24 hours prior to your appointment from our patient portal. We encourage you to respond to our text sent via Klara requesting your insurance cards, ID, and a valid card on file for efficient insurance verification and check-in.

Please arrive 30 minutes ahead of your appointment if you need to fill out your paperwork at the office.

Parking

Patient parking is located on level 1 of the parking garage below the building, access to the garage is located on 25th street. We do not validate parking.

Running late?

We do offer a 30-minute grace period if you are running late.  We will make every effort to accommodate your schedule. Please call us at 404-351-5045 or text us at 470-329-3130 and let us know if you will be late.
A fee of $50.00 may be charged for any same day cancellation or no show.
